Manage medically complex patients transitioning through post-acute recovery within a modern, team-based facility in northern Delaware. Collaborate closely with an interdisciplinary rehab team to coordinate care for full-time rehab hospitalist duties. Required Qualifications
- Board‐certified in PM&R, Internal Medicine, or Family Medicine
- Active or eligible Delaware medical license and DEA
- Experience in inpatient rehab or post‐acute care
- Strong interdisciplinary communication skills
- Comfort managing medically complex patients
