Job Summary
Join Medley Health as a Physiatry / Pain Management Provider, where you will perform evaluations and follow-up visits in skilled nursing and rehabilitation settings. The role focuses on diagnosing and managing musculoskeletal, neurologic, and functional limitations, and developing individualized treatment plans aimed at functional restoration and pain management. Strong interpersonal and documentation skills are essential, along with autonomy in patient management and collaboration with interdisciplinary teams.
Required Qualifications
- MD or DO; Board Certified or Board Eligible in Physical Medicine & Rehabilitation (PM&R) preferred
- Active New York medical license
- Nurse Practitioner (Adult-Gerontology preferred; FNP considered with relevant experience)
- Active New York NP license
Desired Qualifications
- Experience in post-acute care, rehabilitation, pain management, or related settings preferred
- Strong clinical judgment, documentation, and collaboration skills
- Experience in post-acute/SNF care, rehabilitation medicine, pain management, geriatrics, orthopedics, or internal medicine preferred
- Ability to work autonomously and manage a patient panel
- Strong documentation and communication skills
