Job Summary
Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes. Deliver physical therapy services to comply with prescribed physician orders while maintaining discharge care plans through timely documentation of client performance and modifying treatment as needed. Collaborate with the healthcare team, physicians, and family members to coordinate services and educate patients on home exercise plans. Document according to policy to ensure no technical or clinical denials and serve as a clinical resource consultant. Requires a Physical Therapy degree, current unrestricted state license, and experience in home health therapy and OASIS documentation. Full-time Monday through Friday schedule with comprehensive benefits including medical, dental, vision, life insurance, tuition reimbursement, and CEUs.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Experience in Home Health Therapy
- Completion of OASIS documentation
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons
