Partner with the facility's therapy department to provide comprehensive rehab care in a sub-acute setting. Collaborate with the patient's primary care team to ensure optimal outcomes and supervise nurse practitioners and physician assistants delivering PM&R services. Audit documentation to maintain company standards while managing charting remotely. Provide consistent on-site pain management focused on reducing re-admission rates and improving patient functionality in senior living communities. Work within an interdisciplinary team of board-certified physicians and specialists to enhance patient satisfaction.
