Job Summary
Utilize an integrated case management model to assess, plan, implement, and monitor member care while managing whole health services. Observe members, document behavioral concerns in clinical records, and monitor health and safety signs to report issues per policy. Act as an advocate and liaison to identify providers, explore community resources, and develop time-specific service plans. Maintain member records, complete progress notes with appropriate billing codes, and participate in required supervisions. Transport members in company vehicles when necessary and adhere to physical de-escalation requirements. Maintain an approved schedule and follow all organizational policies regarding PHI protection.
Required Qualifications
- Bachelor's degree in a field related to behavioral health, social work etc.
- Associate's degree in a field related to behavioral health, social work etc. AND 2-years related experience
- H.S diploma and 4 -years behavioral health related experience
- Minimum 18 years of age
- DPS Level I fingerprint clearance
- CPR, First Aid, AED certification
- Current, valid Arizona Driver's License
- 39-month Motor Vehicle Report
- proof of vehicle registration liability insurance
- Initial current negative TB test result
- ability and mobility to perform physical de-escalation techniques
- ability to take and pass a physical de-escalation exam
- ability to stand or sit for prolonged periods of time
- ability to occasionally stoop, bend, kneel, crouch, reach, and twist
- ability to lift, carry, push, and/or pull a moderate amount of weight
