Job Summary
Utilize an integrated case management model to assess, plan, implement, and monitor member care, managing whole health needs through the Electronic Health Record. Observe members, document behavioral concerns, and monitor health and safety signs while reporting issues per policy. Act as an advocate and liaison to coordinate services, identify providers, and develop time-specific individual service plans. Maintain member records, complete progress notes with billing codes, and participate in required supervisions. Transport members when necessary and perform physical de-escalation techniques as assigned. This role supports agency goals within Intermountain Centers' statewide behavioral health continuum, offering evidence-based treatment approaches and training.
Required Qualifications
- Bachelor's degree in a field related to behavioral health, social work etc.
- Associate's degree in a field related to behavioral health, social work etc. AND 2-years related experience
- H.S diploma and 4 -years behavioral health related experience
- Minimum 18 years of age
- DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment)
- CPR, First Aid, AED certification, if required (must possess upon hire and maintain throughout employment)
- Current, valid Arizona Driver's License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements, if required
- Initial current negative TB test result, if required (Employer provides)
- Ability and mobility to perform physical de-escalation techniques as necessary, and to take and pass a physical de-escalation exam
- Ability to stand or sit for prolonged periods of time
- Ability to occasionally stoop, bend, kneel, crouch, reach, and twist
- Ability to lift, carry, push, and/or pull a moderate amount of weight
