Job Summary
Pharmacy Technician-Medication Reconciliation (Med Rec) performs comprehensive admission medication history under supervision of a Maryland-licensed pharmacist to create an accurate medication list for effective reconciliation. Responsibilities include knowledge of drug names, dosage forms, herbals and OTC medications; using pharmaceutical terms and abbreviations; performing dosage calculations; maintaining professional communication with patients, families, and health-care staff; ensuring confidentiality and compliance with dress code; participating in training of new staff; meeting accuracy and volume metrics; researching background information (e.g., Surescripts, retail pharmacies, SNFs) to validate patient information; collaborating with ER staff and other teams. Requires high school diploma or equivalent and Maryland-pharmacy registration; PTCB certification is preferred.
Required Qualifications
- High school diploma or equivalent.
- Current registration as a Pharmacy Technician or Pharmacy Intern from the Maryland Board of Pharmacy.
