Job Summary
Pharmacy Technician-Medication History: Gathers a complete and accurate home medication history within 24 hours of admission by interviewing patients or caregivers and coordinating with providers and outpatient pharmacies. Interviews and documents prescription, OTC, and herbal products, verifies allergies, and uses EHR/pharmacy databases to validate histories and identify discrepancies. Documents medication and allergy information in EMR or paper forms, communicates discrepancies to pharmacists/physicians, and collaborates with the healthcare team to support medication reconciliation and patient safety.
Required Qualifications
- High School Diploma or equivalent
- Two years of pharmacy technician experience
- Certified Pharmacy Technician Certification Board (PTCB)
