Pharmacy Technician FT, PT or PRN (Outpatient Department)

Assist with medication reconciliation by interviewing patients and reviewing medication-related documentation to compile medication histories for hospital admissions. Enter compiled history information into the patient's electronic health record to facilitate medication reconciliation by the admitting provider. Perform a variety of functions impacting the medication reconciliation process and patient care, seeking clarification from a pharmacist as needed. Requires minimum of 6 months of current Pharmacy Tech experience and national certification with PTCB or ExCPT upon hire, with registration with the Alabama State Board of Pharmacy required within 30 days of hire.
