Job Summary

Conduct medication reconciliation for high-risk patients by obtaining comprehensive medication histories through interviews with patients and caregivers, communicating with outpatient pharmacies, and updating the electronic health record. Document and report identified discrepancies to the patient's care team, including physicians, nurses, and pharmacists. Participate in Quality Assurance/Quality Improvement projects related to medication reconciliation. Requires CPHT or PTCB certification, a pharmacy technician license, and a high school diploma. Per diem schedule with varied shifts.

Required Qualifications

  • High School Diploma/G.E.D.
  • CPHT - Certified Pharmacy Tech
  • PTCB - Pharmacy Technician Certification Board
  • TCH-Pharmacy Technician License
  • Minimum three (3) years Hospital pharmacy, medical clinic, or retail pharmacy experience
  • Proficiency with the Microsoft Office Suite
  • Demonstrated knowledge of outpatient and retail drugs (prescription and non-prescription) and usual pediatric doses
