Job Summary

Delivers and manages appropriate patient care by determining suitability for home care and directing daily pharmacy department activities to meet pharmaceutical-care needs. Oversees state and federal regulatory requirements, acts as pharmacist in charge per state regulation, and ensures all staff are trained on computer and admixture device use. Coordinates patient services with nursing, delivery, and reimbursement departments while managing and securing all medications, pumps, and medical supplies. Designs and evaluates patient-specific pharmacy care plans, modifies them based on monitoring data, and provides in-service education to physicians, nurses, and technicians. Manages pharmacy staff workload, prioritizes tasks, and coordinates on-call responsibilities.

Required Qualifications

  • Graduate of an accredited school of pharmacy
  • Current, active Pharmacy license as granted by the State Board of Pharmacy
  • At least 3 to 5 years of experience in a clean room or infusion pharmacy setting OR 2 years of experience and one of the following: (a) knowledge of non-sterile compounding, and MTM (b) completion of an ASHP accredited Pharmacy Residency Program, or (c) equivalent experience as approved by the VP of Pharmacy and/or SVP of Clinical Services
  • Access to a reliable means of transportation which will enable the incumbents to make home visits to treat/attend to their patients
  • Valid driver's license
  • Proof of insurance
  • Willing to travel up to 5 % of the time for business purposes (within state and out of state)

Desired Qualifications

  • Post-graduate program in clinical pharmacy service
  • Required certification from the Board in Nutrition Support Pharmacy, Pharmacotherapy or other clinical pharmacy practice area
  • At least 1 year of experience managing inventory and logistics
  • Active member of a state or national professional organization
  • At least 3 years of experience compounding sterile products and managing complex infusion patients
  • At least 1 year of experience overseeing purchasing and inventory process of all pharmaceutical and related substances and supplies
