Pharmacy Specialist-Behavioral Health - Sharp Grossmont Hospital - Variable Shift - Full Time

Review medication orders for accuracy, verify dosing and scheduling for high-cost agents, and provide therapeutic drug monitoring for targeted psychotropic medications. Conduct formal pharmacotherapy consults for pain management, heparin, and pharmacokinetics, documenting recommendations in patient charts. Monitor drug therapy for therapeutic duplications, interactions, and adherence to formulary, while resolving prescribing and formulary issues through direct communication with prescribers. Manage operational tasks including code-blue emergencies, Pyxis system troubleshooting, and medication distribution issues. Reconcile patient medications, provide discharge education, and oversee the hospital Clozapine REMS program. Participate in quality improvement initiatives, adverse drug reaction reporting, and staff training for residents and nurses.
