Job Summary
Pharmacist role providing comprehensive medication management and patient education within El Rio Community Health Center; deliver evidence-based, patient-centered drug therapy recommendations in collaboration with the interdisciplinary care team; evaluate medication orders for accuracy, dosing, interactions, and contraindications; identify and resolve medication-related problems to support patient safety and quality improvement; serve as the primary clinical pharmacy resource for patients, providers, and staff; apply 340B program knowledge to ensure affordable medication access and sound pharmacy operations.
Required Qualifications
- PharmD or equivalent doctoral degree – Required
- Active Arizona Pharmacist License – Required upon hire
- BLS certification including in person skills component – Required upon hire
