Job Summary
Obtain medication and allergy histories for patients admitted to the hospital by interviewing patients, family members, pharmacies, long-term care facilities, and other providers. Communicate this information to pharmacists and healthcare providers to determine treatment plans and medication orders during hospitalization and post-discharge. Maintain registration with the Indiana Board of Pharmacy within 30 days of hire if new, or ensure prior registration is active upon hire. Hold national certification and six months of pharmacy experience. Required Qualifications
- Registration with Indiana Board of Pharmacy within 30 days of hire date if you are a new registrant
- If previously registered, you must have registration upon hire
