Job Summary

Deliver integrated support to patients and pharmacists with a hands-on approach to total quality patient care. Work closely with patients to deliver medication adherence support, educate prospective pharmacy patients on how hospital services improve care, and encourage utilization of specialty clinic services. Seamlessly integrate with the clinic team while providing custom medication management services to hospital patients face-to-face and over the phone. Support outpatient retail and specialty prescription needs, manage outbound check-ups, secure refills, and resolve insurance issues, prior authorizations, and financial assistance. Answer and triage inbound inquiries, perform refill reminder calls under supervision, and ensure accurate documentation in all systems. Identify medications requiring special storage and necessary supplies, manage database applications, and actively participate in process improvement initiatives.

Desired Qualifications

  • CPhT certification
  • 3–5 years of experience as a Pharmacy Technician
  • Knowledge of transplant, hepatitis C, infectious disease (including HIV), and/or oncology
