Job Summary
Provide pharmaceutical care and clinical services to patients in accordance with established protocols, conducting medication therapy management, pharmacotherapy consultations, and drug utilization reviews. Collaborate with healthcare providers and interdisciplinary teams to optimize medication therapy while overseeing clinical activities within the pharmacy department. Ensure compliance with regulatory requirements, accreditation standards, and best practices by participating in quality improvement initiatives to enhance medication safety. Identify opportunities for process optimization to streamline workflows and maintain accurate documentation of clinical activities and outcomes. Prepare reports and presentations as required by pharmacy leadership and regulatory agencies. This role reports to the Pharmacy Director and requires a current pharmacist license with at least one year of hospital pharmacy experience.
Required Qualifications
- Bachelor of Science degree in Pharmacy from an accredited college of pharmacy
- Current pharmacist license to practice in the state required on hire
- Minimum of one (1) year of previous hospital pharmacy experience
- Willingness to commit to a minimum of 3 years of service to receive the signing bonus
Desired Qualifications
- Board Certification in Pharmacotherapy (BCPS) or other relevant specialty certification
- 5+ years as a pharmacist in a critical care hospital
- Experience in a lead or leadership role within a pharmacy setting
