Job Summary
Pharmacist (Temporary Full Time) provides comprehensive pharmaceutical care to inpatients with responsibilities including clinical care in programs such as warfarin dosing and IV to PO conversions, dosing of aminoglycosides and vancomycin, and collaboration with physicians, nurses and allied health team members. The role involves applying pharmacists' clinical scope of practice, delivering teaching to pharmacy residents and students, participating in education rounds and journal clubs, supporting drug distribution, and potentially working across THP sites. Strong clinical judgment, evidence-based decision-making, teaching/mentoring interests, and ability to work across multiple sites are emphasized.
Required Qualifications
- Baccalaureate Degree in Pharmacy or PharmD
- Licensed in Ontario to practice pharmacy
- Completion of hospital pharmacy residency or equivalent experience is an asset
- Strong interpersonal, problem solving, organizational, communication, documentation and critical thinking skills
- Experience in providing exceptional patient centred care
- Experience in hospital pharmacy systems and distribution systems are an asset
- Past experience or a keen interest in teaching and research activities is an asset
- Relevant clinical experience preferred
- Ability to work in a fast paced environment both independently and cooperatively with the team
- Demonstrated self-directed learning and continuing education
- Positive attitude, flexibility to adapt to changing organization needs and accountability
