Job Summary
Review and interpret medication orders from licensed prescribers, assessing drug interactions and allergies while obtaining clarification for incomplete orders. Actively monitor medication therapy in compliance with Collaborative Care Agreements, documenting assessments in patient charts. Verify ingredient compatibility, content, and labeling for compounding and dispensing pharmaceuticals, utilizing barcode technology to ensure accuracy. Supervise interns and technicians, providing direct mentoring and monitoring task completion. Detect and report dispensing variances, adverse drug reactions, and inventory discrepancies in controlled substances. Provide accurate drug information to medical and nursing staff, serve as a preceptor for residents, and maintain current licensure and professional competency.
Required Qualifications
- Graduate from ACPE accredited pharmacy college
- Current US pharmacist license in the state where work is being performed
- Problem solving and critical thinking skills
- Good communication skills
- Familiar with Current Practice Standards (ASHP)
- Previous Hospital Experience
