Job Summary
Pharmacist Clinical Pharmacy Specialist in Ambulatory Clinical Support Team leading chronic disease management under collaborative practice agreements for 5-10 PCPs across primary care practices. Provides face-to-face patient visits, telehealth, and follow-up calls; supports Nurse Clinics; helps develop and implement a new ambulatory care model and service lines; serves as a primary drug information resource, offers in-depth patient counseling, and mentors students/Residents. Responsibilities include ensuring appropriate medication orders, maintaining knowledge of disease states and regulations, leading local/system initiatives, precepting residents, and contributing to training materials and QA processes. Role requires onsite 2-3 days per week with hybrid remote work, and flexible remote options per manager’s discretion; travel to primary care locations as needed and participation in system-wide implementation projects. Education: Doctorate in Pharmacy preferred; Bachelor’s in Pharmacy required. Certifications/licensure: Virginia Pharmacist License, BLS; board-certification in BCACP/BCPS/BCGP preferred; strong emphasis on clinical pharmacy, MTM, population health, and leadership in ambulatory care.

- Virginia Pharmacist License (Required)
- Basic Life Support (BLS) (Required)
- Minimum 3 years of Clinical Pharmacist experience (Required)
- Ambulatory and Clinical Pharmacy Experience (Preferred)
- Residency Trained, PGY1/PGY2 (Preferred)
- Pharmacy Board Certification, such as BCACP, BCPS, or BCGP (Preferred)
