Pharmacist (RFT 1.0 FTE)

Job Summary
Provide leadership, coaching, and mentorship to pharmacy staff while collaborating on efficient, accurate, and timely medication distribution. Lead or support continuous quality improvement initiatives aligned with Ontario College of Pharmacists standards and participate in committees, policy development, and research. Uphold professional Standards of Practice and work collaboratively with healthcare professionals to optimize patient care through medication reconciliation, care planning, safety initiatives, and interdisciplinary rounds. Precept pharmacy learners and contribute to formulary management and drug use evaluations. Ensure all activities comply with staff and patient safety practices and organizational policies.
Required Qualifications
- Bachelor of Science in Pharmacy or entry level Doctor of Pharmacy (PharmD), from a recognized university as determined by the Ontario College of Pharmacists (OCP)
- Registration and in good standing with the Ontario College of Pharmacists (Part A)
- English level A- is mandatory in oral expression, comprehension, reading and writing
Desired Qualifications
- Completion of a hospital pharmacy residency, or a postgraduate clinical pharmacy degree is considered an asset
- Minimum of two years of experience as a hospital pharmacist is considered an asset
- Experience or familiarity in CPOE (computer prescriber order entry), BMV (bedside medication verification, eMedRec (electronic medication reconciliation), eMAR (electronic medication administration records), EMR (electronic medical/ health record) is considered an asset
- Experience working with electronic health record clinical software applications
- Experience in the medication reconciliation process
- Working knowledge of modern hospital care and drug therapy, including knowledge of medication safety risks in health care
- Experience or familiarity in pharmaceutical research in health care settings
- Excellent interpersonal skills
- Ability for self-directed learning and working
- Excellent written and verbal communication and organizational skills
- Bilingual (French/English) is considered an asset
