Pharmacist

Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration routes. Provide clinical recommendations to physicians and staff regarding therapy changes and optimizations. Maintain up-to-date patient profiles including medical history, lab results, and current medications. Ensure laboratory tests for high-risk medications are ordered, reviewed, and communicated. Collaborate with physicians, nurses, and healthcare professionals for clinical and technical support. Document all communications and interventions in patient medical records. Implement policies for safe medication handling, dispensing, and oversight. Ensure compliance with state and federal regulatory requirements. Report medication errors and adverse events to the Pharmacy Manager. Educate clinical team and agency staff on safe medication procedures. Work Monday through Friday with holiday and on-call rotation in a non-retail long-term care environment.
