Job Summary
Evaluate and monitor drug therapy for home care patients in collaboration with physicians, nurses, and other healthcare professionals. Review, verify, and document new patient orders while assessing specific patient, drug, and disease information to identify potential drug-related problems. Process and check medications, supplies, and equipment prior to dispensing, and compound sterile mixtures including parenteral nutrition, antibiotics, and chemotherapy using aseptic technique. Provide consultation and answer inquiries from patients, families, and healthcare professionals regarding drug use. Maintain thorough documentation in patient records and oversee pharmacy technician functions as required. Work Monday through Friday with on-call weekend rotation and holiday coverage.
Required Qualifications
- Bachelor of Science in Pharmacy
- Doctor of Pharmacy preferred
- Registered Pharmacist License in state(s) of practice
- Minimum of two (2) years of recent experience in an acute-care setting
- Minimum of one (1) year of recent experience in home infusion therapy
- Experience with compounding infused drugs
- Proven record of evaluating and monitoring drug therapies for home care patients
- Good organizational and time-management skills
- Excellent verbal and written communication skills
- Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis
- Percentage of Travel: 0-25%
- Ability to push/pull and lift/carry 10-30 lbs
- Ability to sit and type on a keyboard with fingers for constant periods
- Ability to occasionally stand and walk
