Job Summary
Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration routes. Provide clinical recommendations to physicians and healthcare staff regarding therapy changes. Maintain patient profiles, including medical history, lab results, and current medications. Ensure laboratory tests for high-risk medications are ordered, reviewed, and communicated. Collaborate with physicians, nurses, and other professionals for clinical support. Document all communications and interventions in medical records. Implement policies for safe medication handling and dispensing. Ensure compliance with state and federal regulatory requirements. Report medication errors and adverse events to the Pharmacy Manager. Educate clinical team and agency staff on safe medication procedures. Perform other pharmacy-related tasks as assigned.
Required Qualifications
- Bachelor's Degree in Pharmacy
- current and valid pharmacist license in the state of practice
- Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA)
- Proficiency in electronic medical records and pharmacy information systems
- Strong clinical judgement
