Job Summary
Pharmacist role focused on optimizing medication therapy for patient diagnoses, considering age and organ function, and ensuring correct product, dosage, route, and frequency to minimize adverse outcomes. Responsibilities include dispensing per physician orders, ensuring compliance with laws and guidelines, providing drug information to medical/hospital staff, and educating patients and families about therapy. Also monitors Pyxis overrides and reviews related activity; requires strong English proficiency and basic office/computer skills. Per Diem employment at Palomar Health location in Poway, CA. Licensure as a CA Registered Pharmacist (RPh) is required; PharmD preferred with potential Board Certification in Pharmacotherapy. Familiarity with pharmacy operations, residency experience, and continuing education are assets.
Required Qualifications
- Current CA Registered Pharmacist (RPh)
- PharmD preferred
- Board Certified Pharmacotherapy Specialist preferred
- Minimum Education: As required by certification and/or licensure
- Minimum Experience: 2 years or 1 year Residency
- Preferred Experience: 3 years or 1 year Residency with additional year of experience
