Job Summary

Verify medication orders, assess optimal drug selection, and design dosage regimens while monitoring drug therapy for safe, timely, and cost-effective outcomes. Interact with patients, families, and healthcare providers to ensure continuity of services and desired patient outcomes. Supervise pharmacy technicians and assume essential pharmacy management responsibilities in your absence. Lead pharmacy services in clinical or operational areas, providing expertise and oversight to designated staff. This per diem role at John Muir Health supports ambulatory care clinics including Palliative Care, Internal Medicine, and Refill Clinics.

Required Qualifications

  • School of Pharmacy - Accredited by ACPE
  • Completion of ASHP Accredited General Practice Residency (PGY1)
  • RPH Registered Pharmacist - California Board of Pharmacy
  • BLS Basic Life Support - American Heart Association
  • PALS Pediatric Adv Life Support - American Heart Association (for staff working in Pediatric Unit Based shifts and Emergency Dept Unit Based Shifts)
  • Full understanding of pharmacy distribution and workflow
  • Well-developed written and verbal communications skills and interpersonal skills
  • Must have knowledge of hospital clinical and ancillary services operating procedures and how departments interrelate
  • Ability to manage projects, meet deadlines and establish and maintain effective working relationships with all levels of hospital staff and physicians
  • Competency in the use of PC hardware and software
  • Completion of a Pharmacy - American Society of Healthy-System Pharmacists (ASHP) - Accredited Residency - PGY1 (for Ambulatory Care)
  • Prior experience in Inpatient pharmacy or Infusion Center setting (for Oncology)
  • Prior experience in Oncology Pharmacy setting (for Oncology)

  • Completion of a PGY-1 residency with pediatric and neonatal pharmacy services as part of the residency training or have prior experience in the area of pediatric and neonatal pharmacy services (for Pediatric)
  • Completion of ASHP Accredited General Practice Residency (PGY1) (for Critical Care)
  • Completion of ASHP Accredited Critical Care Residency (PGY2) or Critical Care experience (for Critical Care)
  • Critical Care Board certification (BCCCP) or Critical Care experience (for Critical Care)
  • Must have a strong knowledge of critical care medicine and therapeutics (for Critical Care)
  • Willingness to rotate to other pharmacy areas (for Critical Care)
  • Must have a basic knowledge of chronic disease states and common treatments (for Ambulatory Care)
  • Must be able to educate, monitor, order, and follow labs for medications related to disease state management (for Ambulatory Care)
