Job Summary
Identify medication-related problems including appropriateness of drug, dose, route, frequency, and duration; minimize therapeutic duplication; and prevent drug-drug, drug-food, drug-lab, or drug-disease interactions. Monitor patient response to therapy and provide accurate drug information to multi-disciplinary team members and patients. Follow and manage pharmacy protocols for heparin, warfarin, pharmacokinetics, IV to PO transitions, and renal dosing. Participate in quality improvement projects and lead development of specialty pharmacy services under pharmacy leadership. Act as a resource in specialty areas and maintain consistent documentation of consults, interventions, adverse drug reactions, and medication variances. Work 40 weekly hours at the Monument Health Neuroscience Center.
Required Qualifications
- Bachelors in Pharmacy
- Pharmacist - South Dakota Board of Pharmacy
- Advanced Cardiac Life Support (ACLS) Certification - American Heart Association (AHA)
- Basic Life Support (BLS) Certification - American Heart Association (AHA)
- 1+ years Pharmaceutical Experience
- 3+ years Hospital Experience
- 3+ years Clinical Experience
- Exerting up to 20 pounds of force occasionally
- Exerting up to 10 pounds of force frequently
- Exerting a negligible amount of force constantly
Desired Qualifications
- Doctorate in Pharmacy
- Post-graduate Year 1 (PGY-1) - American College of Clinical Pharmacy (ACCP)
- Pediatric Advanced Life Support (PALS) Certification - American Heart Association (AHA)
