Job Summary
Provide diabetes counseling, training, and education while conducting comprehensive medication assessments and therapy management. Deliver personalized guidance on medication use, lifestyle strategies, and adherence to build trusted patient relationships. Collaborate with prescribers and allied healthcare professionals to ensure coordinated care, documenting all consultations in compliance with standards. Stay current with diabetes guidelines and support pharmacy operations including dispensing and administration. Work full-time Monday through Friday from 8:00am to 6:00pm with no late nights or weekends.
Required Qualifications
- OCP Part A licensed pharmacist in good standing with current OCP Injection Certification
- Must have previous retail pharmacy experience
- Ability to travel locally to other LMC sites
Desired Qualifications
- 2-3 years of Ontario pharmacy practice and diabetes education experience preferred
- Certified Diabetes Educator (CDE) certification preferred
- Kroll proficiency and EMR experience an asset
- Excellent verbal and written communication skills
- Excellent interpersonal, organizational and time management skills
- Interest in helping people with diabetes improve their health and wellbeing
- South Asian language competency (Punjabi, Hindi) preferred
