Pharmacist

Review and evaluate patient medication profiles to ensure therapy appropriateness, consistency with formulary guidelines, and achievement of therapeutic goals. Perform medication therapy management, monitor adverse drug events, and document interventions in the electronic health record. Oversee preparation of sterile and non-sterile compounded doses while ensuring accurate dispensation during assigned shifts. Provide education to patients and healthcare staff, serve as a preceptor for pharmacy students, and support new pharmacy staff training. Maintain licensure, complete mandatory safety modules, and resolve reimbursement eligibility issues with third-party payers.
