Job Summary
Review and evaluate patient medication profiles to ensure therapy appropriateness, consistency with formulary guidelines, and alignment with collaborative practice protocols. Document interventions, monitor adverse drug events, and provide accurate medication information to staff and patients using electronic health record tools. Oversee medication preparation, compounding, and dispensation while supervising technicians and precepting pharmacy students and residents. Foster teamwork through open communication, educate patients on drug therapy, and resolve reimbursement issues with third-party payers. Maintain clinical knowledge, complete mandatory safety modules, and follow all controlled substances policies to ensure patient and facility safety.
Required Qualifications
- Current Registered Pharmacist License (RPh) for applicable state required
- BLS for Provider certification from the American Heart Association required within 3 months
