Job Summary
Pharmacist role providing safe and appropriate pharmacy services in home infusion, coordinating patient services, assessing therapy appropriateness, dispensing medications and supplies, maintaining patient medication profiles, documenting patient progress and education, ensuring compliance with state/federal regulations and USP standards, supervising pharmacy support personnel, participating in patient care conferences, and contributing to QA/QC and on-call processes. Requires a Doctor of Pharmacy or PharmD (or MS in Pharmacy as desirable) and active state licensure; minimum 2 years acute-care or home infusion experience, strong communication and leadership skills, and ability to collaborate with physicians/nurses for optimal patient care.
Required Qualifications
- Bachelor of Science in Pharmacy (Doctor of Pharmacy or Master’s of Science desirable)
- Currently licensed as a registered pharmacist in the state of employment
- Minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy
- Experience in intravenous admixture services and intravenous therapy
- Ability to evaluate and monitor drug therapy collaboratively with physicians and nurses
- Ability to work effectively with others to provide safe and effective drug therapy
- Strong problem solving and supervisory skills
- Flexible with work schedule
- Good organizational and time-management skills
- Participation and membership in professional societies desirable
- Excellent verbal and written communication skills
- Dependability in attendance and performance
