Job Summary
Conduct medication distribution, compounding, and drug information services for inpatient patients. Manage formulary management, drug usage, adverse drug reactions, and medication error prevention while collaborating with nursing and pharmacy staff. Perform medication use evaluations, develop clinical programs, and provide education and resource materials. Maintain knowledge of regulatory standards and ensure patient confidentiality. Requires current New York State registration, Pharm.D. degree, and one year of ambulatory care experience. Pay range $55.00–$65.00; schedule includes days, evenings, nights, and weekends.
Required Qualifications
- Current New York State Pharmacist registration
- Accredited continuing education as required/defined by New York State
- Bachelor's degree in pharmacy or Pharm. D from accredited Pharmacy program
- Possesses, maintains and enhances knowledge of all accrediting and/or regulatory agency standards (i.e. Joint Commission, DOH, etc.)
- Actively guards the confidentiality of sensitive information including but not limited to the patients, staff and the health system
- Demonstrates the knowledge & skills necessary to provide care appropriate to the age of the patient
- Possessing an in-depth knowledge of medications and their uses
- Flexibility is required
