Job Summary
Disburse medications prescribed by physicians and provide counseling on drug therapy, dosing, interactions, and side effects to patients and providers. Verify prescriptions for appropriateness, evaluate food interactions, and make decisions regarding generic and therapeutic substitutions per formulary requirements. Review and ensure the accuracy of drug therapy while collaborating with the health care team to communicate patient understanding of desired outcomes. Maintain compliance with Arizona State Board of Pharmacy regulations, Joint Commission standards, and 340B Drug Pricing Program rules. Provide operational oversight of pharmacy staff, train interns, and enforce safety, infection control, and loss prevention protocols. Ensure patient confidentiality and maintain a clean, hygienic work environment. Requires a current Arizona Pharmacist License, BLS certification, and Level I fingerprint clearance.
Required Qualifications
- A Diploma from an accredited College of Pharmacy
- A current Pharmacist License in accordance with the Arizona State Board of Pharmacy requirements
- Current certification in Basic Life Support (BLS) for health care providers
- Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment
- Reliable transportation that can meet any operational reassignments of the organization during the workday
- Valid driver's license
- Compliance with Arizona vehicle insurance requirements
- Current valid and in good standing Level One (1) Non-IVP Fingerprint Clearance card (must be received within 30 days after the new employee hire date or transfer date)
- Influenza vaccination status (subject to exemptions and accommodations when required by law)
- Undergo drug testing prior to employment
- Subject to post-accident, reasonable suspicion, return to duty and follow up drug and alcohol testing in compliance with Federal and State regulations for alcohol and controlled substance testing
Desired Qualifications
- Board Certified by the Board of Pharmacy (Ambulatory Care or Pharmacist Specialist) or Certified Diabetes Educator
- Bilingual (English/Spanish) with the ability to speak, read and write in both languages
