Pharmacist Consultant LTC

Job Summary
Conduct monthly medication regimen reviews for long-term care facility residents, collaborating with physicians and nursing staff to optimize drug therapy outcomes. Identify and resolve drug-related problems including interactions, duplications, and contraindications while ensuring regulatory compliance with state and federal guidelines. Provide clinical recommendations and pharmacotherapy consultations to facility staff, and participate in quality assurance and performance improvement initiatives. Assist in developing and updating facility medication policies and procedures. Requires an active California Pharmacist license, PharmD degree, and OBRA knowledge; remote position with 401(k), dental, health, and vision insurance.
Required Qualifications
- Active and unrestricted Pharmacist license in the state of California
- Doctor of Pharmacy (PharmD) degree or equivalent from an accredited institution
- Strong knowledge of OBRA regulations, CMS guidelines, and LTC compliance standards
- Excellent communication and interpersonal skills for working with multidisciplinary teams
- Highly organized, detail-oriented, and able to manage multiple facility accounts
Desired Qualifications
- Prior experience in long-term care (LTC), geriatric, or consultant pharmacy preferred
- Consultant Pharmacist certification (BCGP or similar) is a plus
