Pharmacist Consultant - CarelonRx Call Center

Pharmacist Consultant at CarelonRx Call Center with hybrid/virtual work and required in-person training. Responsible for supporting disease management programs and physicians’ care plans through drug-therapy evaluation and consultation for program participants. Activities include evaluating member Rx needs and therapies via medication history, ensuring appropriate use of prescriptions, assessing and addressing barriers to compliance, counseling physicians and members, and serving as a clinical-resource across disease-management and other clinical areas. Requires BA/BS in Pharmacy with 2+ years of clinical experience and an active pharmacist license; PharmD preferred. Work schedule is second shift (2nd Shift) with potential in-person onsite requirements and occasional non-standard hours; hybrid/virtual work policy applies, with weekly onsite presence at Elevance Health locations.
