Job Summary

Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration, and stability. Review drug regimens and clinical indicators monthly, reporting irregularities to nursing, administrators, and physicians with written findings. Document all communications with healthcare teams, patients, and families. Maintain knowledge of pharmacy laws and federal/state regulations concerning long-term care while working cooperatively as a team member. This remote role covers specific listed territories and requires immunization certification. Candidates must reside within those regions, work weekends or outside normal hours, and possess a current pharmacy license with one year of acute, long-term care, or home health experience preferred.

Required Qualifications

  • Bachelor's degree from an accredited School of Pharmacy or Pharmacy Doctorate
  • Current license
  • Ability to work weekends or outside of normal business hours when needed
  • Ability to lift patient charts repetitively
  • Ability to stand for long periods of time
  • Good customer service skills
  • Ability to learn and understand state and federal regulations for long-term care
  • Understand of formulary management

Desired Qualifications

  • Dual license
  • One-year experience in acute, long-term care or home health
  • Consultant or Clinical Pharmacist experience
  • IV clinical experience
