Pharmacist Clinician - Part-time

Conduct comprehensive medication reviews, manage chronic and acute disease states, and optimize pharmacotherapy for direct patient care. Evaluate laboratory values and clinical data to adjust medication therapy plans, while writing, renewing, and discontinuing prescriptions with full documentation in the electronic health record. Collaborate with physicians, nurses, and other healthcare professionals to improve patient outcomes, participate in interdisciplinary care conferences, and support quality improvement initiatives through medication safety and utilization reviews. Perform essential staff pharmacist duties including prescription verification, patient counseling, and compliance with regulatory standards. Obtain New Mexico pharmacist licensure and complete a PGY-1 residency within six months of employment.
