Job Summary
Verify medication orders, compound and dispense pharmaceuticals, and conduct home medication reconciliation, chart review, and patient counseling in accordance with federal, state, and local regulations. Deliver evidence-based clinical consultation to practitioners, make pharmacotherapy recommendations through multi-disciplinary collaboration, and supervise pharmacy technicians and interns. Document all medication-related plans and interventions in the electronic medical record while maintaining a constant state of alertness. Work variable shifts totaling 36 hours per week, including days, evenings, and occasional overnight coverage, with an every-third-weekend rotation.
Required Qualifications
- Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy
- Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR)
- Pharmacist, Registered - Missouri Division of Professional Registration
- Pharmacist - Oklahoma State Board of Pharmacy
- Pharmacist - Wisconsin Department of Safety and Professional Services
- Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment
