Job Summary
Coordinate medication management for patients enrolled in value-based clinical programs and pharmacy services, performing clinical assessments to optimize therapies and manage physician-prescribed regimens. Collaborate with the patient care team to promote safe and effective medication use across telephone, outpatient pharmacy, and home visit settings. Lead precepting of pharmacy residents, students, and interns while managing operational duties for diverse clinical programs. Requires a Doctor of Pharmacy degree, Maryland pharmacist license, and one year of direct patient care experience.
Required Qualifications
- Graduation from an accredited College of Pharmacy resulting in a Doctor of Pharmacy degree
- Equivalent two years of experience
- State of Maryland pharmacist license
- Certified by the Maryland Board of Pharmacy to provide vaccinations
- Active CPR certification
- Minimum one year direct patient care experience as a pharmacist
Desired Qualifications
- Completion of an ASHP accredited residency program
- Advanced certification in area of practice (e.g. BCPS, BCACP, AAHIVP, CSP, etc.)
- Certified by the Maryland Board of Pharmacy to provide vaccinations preferably upon hire
