Job Summary
Conduct telephonic consultations with patients regarding specialty medication use, disease state management, and side effect mitigation. Perform medication reconciliation, screen for drug interactions, and interpret prescriptions to expedite order processing. Provide patient education on administration, storage, and therapeutic appropriateness while documenting interventions in pharmacy software. Contact prescribers for clarification, verify controlled substances, and work with physicians to convert prescriptions to generics or biosimilars. Back up team members, train less experienced pharmacists, and report medication errors per company policies.
Required Qualifications
- Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree
- Current, unrestricted pharmacist license in state of residence
- Ability to consistently interact cooperatively and respectfully with other employees
- Strong clinical background
Desired Qualifications
- At least two years of experience as a practicing licensed Pharmacist
- Experience and/or strong understanding of Specialty Pharmacy medications
