Job Summary
Review and dispense all doctors' medication orders for appropriateness, identify and resolve drug-related problems, and monitor drug therapy for effectiveness and adherence to evidence-based guidelines. Conduct medication reconciliation, follow up on medication incidents to promote safe practices, and respond to drug information questions while providing direct or functional supervision of technician staff. Perform other duties consistent with the job classification as required. This centralized inpatient pharmacy support role is based at Toronto General Hospital within a fast-paced environment requiring close collaboration with healthcare teams.
Required Qualifications
- Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent
- Certificate of competency from the Pharmacy Examination Board of Canada
- Active registration with the Ontario College of Pharmacists (Part A)
- Hospital pharmacy practice experience with patient-centered care experience in surgery
- Active registration with OCP (Part A)
- Experience in an acute-care hospital pharmacy or ambulatory pharmacy setting
Desired Qualifications
- A Faculty of Pharmacy University appointment
- A hospital pharmacy residency certificate
- An active membership in CSHP and other professional societies
- Hospital Pharmacy residency
- Clinical experience with patient-centered care in oncology
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies
- Excellent verbal and written communication skills
- Excellent customer service and interpersonal skills
- Excellent organizational and time management skills
