Job Summary
Clinical Pharmacist responsible for providing pharmaceutical care and resolving drug-related problems through medication profile review, patient assessment, and developing/implementing Pharmaceutical Care Plans. Delivers drug information to physicians, nurses, and allied health professionals; participates in committees; conducts Drug Utilization Evaluation projects; provides in-services to professional staff; mentors pharmacy students and new staff; ensures prescriptions are validated and dispensed accurately; supports medication safety and collaborates with the health care team to resolve discrepancies. Requires PharmD (preferred), licensed pharmacist in Ontario, and relevant hospital pharmacist experience; strong knowledge of therapeutic drug monitoring and clinical pharmacology; ability to lead, educate, and participate in quality improvement and guideline development.
Required Qualifications
- Doctorate of Pharmacy degree (PharmD) preferred
- Licensed as a Pharmacist with the Ontario College of Pharmacists
- Certification of competency from the Pharmacy Examination Board of Canada
- Completion of an accredited Hospital Pharmacy Residency Program or equivalent
- Strong knowledge in therapeutic drug monitoring
- In-depth understanding of drugs and clinical effects
- Experience in hospital setting with direct pharmaceutical patient care (preferred)
- Ability to work within multidisciplinary teams
- Proven ability to develop and implement drug therapy guidelines
- Experience with medication use evaluations and quality improvement initiatives
- Mentorship and supervision of students and new staff
