Job Summary
Clinical Pharmacist role providing pharmaceutical care to optimize drug utilization and patient outcomes. Responsibilities include performing medication profile reviews, resolving drug-related problems, developing and implementing Pharmaceutical Care Plans, providing drug information to physicians and other health-care professionals, collaborating with the medical team to develop and monitor therapeutic plans, participating in Drug Utilization Evaluation projects, contributing to quality improvement initiatives and committees, mentoring pharmacy students and new staff, ensuring accurate dispensing and medication safety, and documenting professional practice activities. Requires Doctorate of Pharmacy (preferred) and licensure in Ontario; strong pharmacology knowledge, therapeutic monitoring, and excellent communication and teamwork skills. Location at Rideau Lakes area (Kemptville), Ontario.
Required Qualifications
- Doctorate of Pharmacy degree (PharmD) or equivalent considered; Licensed as a Pharmacist with the Ontario College of Pharmacists; Certification of competency from the Pharmacy Examination Board of Canada; Completion of an accredited Hospital Pharmacy Residency Program or equivalent preferred; Working hospital pharmacy experience preferred
