Job Summary
Provide clinical pharmacy services to patients, including pharmaceutical care to optimize drug utilization, identifying and preventing drug-related problems, reviewing medication profiles, patient assessment, developing and implementing Pharmaceutical Care Plans, and delivering drug information to healthcare providers. Collaborate with physicians and other health care professionals to develop, implement, and monitor therapeutic plans; participate in medication use evaluations, quality improvement initiatives, and committees; mentor pharmacy students and new staff; ensure prescriptions are validated and dispensed accurately, and address medication safety concerns in collaboration with the health care team.
Required Qualifications
- Doctorate of Pharmacy degree (PharmD) preferred
- Licensed as a Pharmacist with the Ontario College of Pharmacists
- Certification of competency from the Pharmacy Examination Board of Canada
- Completion of an accredited Hospital Pharmacy Residency Program or equivalent experience preferred
- Strong knowledge in therapeutic drug monitoring
- Good understanding of clinical pharmacist’s role within the multidisciplinary team
