Identify medication-related problems regarding drug appropriateness, dose, route, frequency, and duration while minimizing therapeutic duplication and preventing drug-drug, drug-food, or drug-disease interactions. Monitor patient response to therapy and provide accurate drug information to multi-disciplinary team members and patients. Follow and manage pharmacy protocols for heparin, warfarin, pharmacokinetics, IV to PO transitions, and renal dosing. Participate in quality improvement projects and lead development of specialty pharmacy services under pharmacy leadership guidance. Maintain consistent documentation of consults, interventions, adverse drug reactions, and medication variances relative to peers. Work 40 weekly hours at Monument Health Rapid City Hospital CCI Pharmacy.
