Identify medication-related problems including appropriateness of drug, dose, route, frequency, duration, and prevent drug-drug, drug-food, or drug-disease interactions. Monitor patient response to therapy and provide accurate drug information to multi-disciplinary team members and patients. Follow and manage pharmacy protocols for heparin, warfarin, renal dosing, and therapeutic interchange. Participate in quality improvement projects and lead development of specialty pharmacy services under pharmacy leadership guidance. Document consults, interventions, adverse drug reactions, and medication variances consistently relative to peers. Work 40 weekly hours in the Rapid City Hospital CCI Pharmacy department.
