Job Summary
Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration routes. Provide clinical recommendations to physicians regarding therapy changes and optimizations. Maintain patient profiles, order and review lab tests for high-risk medications, and document all interventions in medical records. Collaborate with healthcare staff on clinical support, enforce safe medication handling policies, and ensure regulatory compliance. Report medication errors and adverse events as required. This role supports long-term care and senior living clients in a non-retail environment.
Required Qualifications
- Bachelor's Degree in Pharmacy
- current and valid pharmacist license in the state of practice
- Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA)
- Proficiency in electronic medical records and pharmacy information systems
- Strong clinical judgement
