Job Summary

Evaluate physician medication orders for drug appropriateness, dosage, interactions, and route of administration. Provide clinical recommendations to physicians and healthcare staff regarding therapy changes, maintain up-to-date patient profiles including medical history and lab results, and ensure required laboratory tests are ordered, reviewed, and communicated. Collaborate with physicians, nurses, and other professionals to support clinical and technical needs, document all communications and interventions in medical records, and enforce policies for safe medication handling and dispensing. Ensure compliance with state and federal regulatory requirements, report medication-related errors and adverse events to the Pharmacy Manager, and educate the clinical team on safe medication procedures. Work varying shifts from 7am to 11pm in a non-retail long-term care environment.

Required Qualifications

  • Bachelor's Degree in Pharmacy
  • current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA)
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgement
  • effective written and verbal communication
