Job Summary
Pharmacists work independently with their patients to meet therapeutic goals while also being a vital member of a multidisciplinary team. As the Medication Management Clinic grows; Pharmacists have opportunities to contribute to the development of new services and find a role within the clinic that best suits them. Responsibilities include assessing, documenting, and performing physician consultations (excluding total parenteral nutrition) with medication dosing expertise, performing home medication reconciliation, providing education presentations to pharmacy staff and other staff, and supervising personnel as appropriate. Strong emphasis on confidentiality and professional communication. Relocation is offered.
Required Qualifications
- Minimum Education: B.S. Pharmacy or Pharm D; or Master of Pharmacy
- Continuing education as required for licensure by TSBP
- Minimum Work Experience: Previous hospital experience with clinical responsibilities
- Required Licenses/Certifications: Active registration as a licensed pharmacist in the state of Texas
- Sterile Compounding Certification
- ACLS or PALS
- Registration as a preceptor with TSBP
- Perform home medication reconciliation
- Assess; document; and perform physician consultations (except total parenteral nutrition) including all medication dosing consults
- Provide education presentations to pharmacy staff and other staff upon request
- Ability to effectively direct and/or supervise personnel as appropriate
- Ability to handle confidential information
- Ability to maintain professional appearance and decorum at all times
- Ability to demonstrate excellent oral and written communication skills
