Job Summary
Coordinate patient transition management for chronically ill individuals moving from hospital to home-based intensive care with intravenous therapies. Manage logistics including prescription handling, ordering systems, and internal service coordination while overseeing a territory of 20–25 patients. Deliver direct patient care through nutritional counseling, port management, and individual pharmaceutical support, alongside training patients, family members, and nursing agencies on infusion pump operation. Plan routes and schedule appointments independently, reporting progress to team leadership. Requires certified nursing background with experience in patient care and self-organization for daily travel duties.
Required Qualifications
- Abgeschlossene Berufsausbildung in der Behandlungspflege als examinierte Krankenschwester, Gesundheits- und/oder Krankenpfleger (m/w/d)
- Berufserfahrung in der Patientenversorgung
- Fähigkeit der Selbstorganisation und des Managements Ihres eigenen Gebietes (ca. 20-25 Patienten)
- Bereitschaft zur Tages-Reisetätigkeit (Übernachtungstouren sind nicht erforderlich)
- Gute kommunikative Fähigkeiten sowie ein menschlich-verbindliches Wesen mit einem den Situationen angemessenen Einfühlungsvermögen
Desired Qualifications
- Idealerweise abgeschlossene Weiterbildung im Bereich der Intensivmedizin, Anästhesie, Onkologie oder Palliativmedizin
