Job Summary
Conduct personalized support for clients with psychiatric vulnerabilities or addiction, focusing on skill development, self-reliance, and relapse prevention. Coordinate care plans and act as the primary contact point for social and medical networks, including GPs and housing corporations. Manage daily logistics such as cooking, grocery assistance, and housing maintenance while accompanying clients to appointments and day activities. Operate across multiple Utrecht and Lekstroom locations with flexible hours from Monday to Sunday, rotating between personal guide and support roles. Collaborate with colleagues to ensure continuous, methodical care that fosters client autonomy.
Required Qualifications
- Relevant diploma op hbo-niveau (zoals bijvoorbeeld Social Work, SPH, MWD)
- Relevant zorg diploma op mbo niveau 4 (zoals MZ, Social Work, verpleegkunde) of ben je in het bezit van je propedeuse op hbo-niveau
- Aantoonbare werkervaring in de psychiatrie en/of de verslavingszorg is een pré
- Je bent inzetbaar op diverse locaties en kan hierin goed schakelen tussen verschillende doelgroepen
- Je kunt je observaties en bevindingen goed verwoorden en delen met je collega's
- Kennis van de diverse culturen die voorkomen in Nederland is een pluspunt
